Booking Appointments

In order to reserve an appointment a credit card must be placed on file. This is required as  part of my late cancel/No Show policy.

You will NOT be charged until after the session is completed if there is a balance owed. All clients will be charged  if the cancellation policy is violated.  

Intake Form

For new clients visiting Lotus Massage and Rehabilitation for the first time, I would like to say thank you for choosing  our services.

 

Once you have booked the appointment, you will  receive an email from me with an intake form link attached to the bottom. This form will need to be filled prior to arriving for your appointment. Failure to complete this form prior to your arrival, will cut into your massage time as there are important questions about your health that will need in advance of performing your services.

 Important Information when booking:

 

• Late Reschedule /Cancellation & No-Show Policy – I am a By Appointment Only provider. This means that once you reserve a service on my schedule the time belongs only to you. Out of consideration for my Schedule and other clients waiting to reserve services, if you cancel in less than 24 hours you will be charged a cancellation fee of 50% of the value of your treatment booked.  In case of an Emergencies or illnesses this may exempt you from this fee so long as you notify me by (1) calling and leaving a voicemail or (2) sending a text message as soon as you can upon discovering you will need to cancel/reschedule your appointment.

• Late Policy- If you are late for your appointment for more than 10 minutes past your scheduled time, out of respect for the clients scheduled behind you, your session will be cut short. If you are more than 10 minutes late for your appointment, you will automatically require rescheduling your session. If your appointment start time was delayed for any reason because of me, I will honor your time and complete the full hour because I am the one at fault.

• By signing this release, I hereby waive and release my therapist from all liability, past, present, and future relating to massage therapy and bodywork.
